Re-Enrollment Process

Complete Re-enrollment form and pay registration and enrollment fee.
Signature on current year fee sheet.
Signature on the Patron Agreement form.
Review Fee Sheet.
For students who will be driving to campus – copy of driver’s license and insurance card

Once your application has been processed the following documents must be provided

Certified copy of student’s birth certificate
Copy of Social Security Card
Updated Immunization Compliance Form 121
Copy of last report card or progress from previous school
Copy of guardianship documents if applicable
For students who will be driving to campus – copy of driver’s license and insurance card
